Pension Paperwork - James Madison Phillips (Civil War Veteran)
War Department, adjutant General's Office, Washington, Nov. 14, 1883 -- Respectfully returned to the Commissioner of Pensions. James M. Phillips, a ___ [hard to read -- may be private retired] of Company "A" 16" Regiment Wis. Volunteers, was enrolled on the 30 day of Mar, 1864, at Madison, for ___ [illegible to me], and is reported: On roll of the Co. for War and Apl '64 [April ?] present. Same report to Aug. 31, '64. Sept. & Oct. '64 absent sick and so reported to April 30, '65. May & June '65 present. Mustered out with Co. July 12, '65 at Louisville, Ky. Ret. for May '64 absent, left at Hunstville, Ala. July '64 and reported absent. Nature of sickness not stated Regimental hospital records on Company ____ [illegible] reports are not on file.
